So, let's talk about the basic form of glandular repositioning. Allow me to break it down for you. Understanding this fundamental technique is key to recognizing the differences between various methods and how they've evolved over time. Back when I was in residency, and even in 2012 when I attempted my first repositioning,

I was introduced to the inferior pedicle type.



It's considered the starting point, the foundation, if you will, of glandular repositioning. In breast lift surgery, we're essentially moving that hefty glandular tissue from the lower part of the breast upward. This repositioned tissue keeps the blood flowing from below, hence the term "inferior pedicled flap." Now, if we're preserving blood flow from above, that's what we call the superior pedicled flap—a technique I also use. When doing the glandular repositioning with an inferior pedicled flap, how we secure that tissue post-surgery plays a huge role in the outcome.

Some docs might say lifting the glandular tissue doesn't do much, and that's likely because of how we're securing it.









Breast tissue is heavy, and without strong ligaments to support it, no matter how high we lift it, if the fixation isn't solid, it can sag easily.

That's why I use the pectoralis major muscle for fixation during glandular repositioning.


We create a loop-like structure with that muscle and pass the tissue through it before securing it.








This way, the tissue is supported by the muscle's strength.

I often explain it to patients like hanging laundry on a clothesline, 'cause that's the most effective way to visualize it.


The breast tissue deals with a lot of movement, whether it's arms, walking, or lying down.


To handle all that, this kind of fixation is the best bet.

Today, I've covered the basics of the glandular repositioning I do.


Making the breasts look fuller and more prominent means ensuring good blood supply and strong, lasting fixation.
