Understanding

the Attraction of a Fuller Chest

and the Issue of Upper Bulge

Hi Doctor :) Is a fuller chest considered more attractive?

Dr. Ock Jae Jin: Most people do desire a fuller, more voluminous chest.

Even if there is an upper bulge (upper part protruding too much)?

Ah, not necessarily. First, we need to understand what an upper bulge is.

What causes an upper bulge?

When you undergo breast surgery, swelling often occurs.

This swelling tends to move to areas with less pressure,

like the upper part of the chest,

making it appear swollen.

Another cause could be using implants that are too protruding.

If the skin is pushed by the implants,

it can cause both the upper and lower parts to stick out.

In such cases, this is a temporary upper bulge.

As the skin stretches over time, it usually improves.

How can we distinguish between a temporary and a permanent upper bulge?

  1. High Starting Point of the Chest: If the starting point of the chest is too high, the implant is placed too high.

  2. Distinct Boundary of the Bulge: If the boundary of the bulge is very defined, it means the implant space is too narrow.

Temporary upper bulges tend to improve over time,

usually within six months.

Comparing the shape in the first and second months and

noting any changes is very important.

Can we prevent an upper bulge before surgery?

Choosing the right implant is crucial.

If the upper chest boundary is too high,

it might mean you chose an implant with a large diameter,

leading to an upper bulge.

Additionally, if the chest is narrow

but you want a larger chest,

the soft tissue and skin need to stretch to accommodate the implant. Initially,

before the skin stretches,

it may press the implant tightly,

causing the upper and lower parts to bulge.

How to prevent an upper bulge?

  1. Implant Selection: Decide on the appropriate diameter and projection of the implant.

  2. Surgical Technique: Determine the position where the muscle will press on the implant during surgery. Proper design can prevent an upper bulge.

  3. Implant Position: Adjust the implant's position slightly lower if the upper chest protrudes. The height, chest width, and chest length of the person receiving the surgery are factors to consider.

How to address an upper bulge after surgery?

The implant space shouldn't be too narrow or too wide.

If it's too narrow,

the implant can't move and is tightly wedged in.

When the tissue presses strongly from above,

an upper bulge becomes more pronounced.

In such cases, the implant space should be widened,

or the muscle should be repositioned higher to reduce the pressure from top to bottom.

If the implant is too high, it needs to be lowered,

which means creating more space below the implant.

Sometimes, a capsule contracture occurs,

where the capsule around the implant tightens,

causing a flat implant to become round like a ball,

exacerbating an upper bulge.

In such cases, a revision surgery for capsule contracture may be necessary.

These methods should be chosen according to the situation for the surgery.

Why is ultrasound important for an upper bulge?

An upper bulge is typically assessed visually.

Ultrasound helps to determine the cause.

We conduct ultrasound exams after surgery,

and if necessary actions are identified,

taking those actions immediately is crucial for achieving

stable and good results.

The most important thing is to prevent an upper bulge in the

first place.

This is determined before the surgery.

Meeting a good surgeon, making a solid plan,

and aiming for stable surgical outcomes is key.
